Region BOM not operational

Issue

Deployments have been stuck in bom region since ~10:35 UTC today. New machines get created but never progress past the created state — the image pull starts but never completes, looping every ~5 minutes indefinitely.

What I observed

  • Machine stuck in replacing state, then destroyed and redeployed
  • Every new machine lands in created state and repeatedly pulls the same image digest (sha256:d78c6051230189ace5429a7a3500b87a5d656aea2bc091e6602268f4ca545ad4) every ~5 minutes with no progress
  • Error: machine is in a non-startable state: replacing
  • Error: could not find a good candidate within 40 attempts at load balancing — app had zero healthy instances
  • This affected 3 different machines over several hours
