Error on `fly ext sentry create`

For one of my apps (tt-dev-coach-processing), I get the following error when trying to run fly ext sentry create.

Error: input:3: createAddOn Validation failed: Name has already been taken

Looking over the forums it looks like maybe Fly will need to clean something up for me? The command succeeded for my other apps.
