CD - Can't get the Github Branch Preview Workflow to work

I have been following the guide here Git Branch Preview Environments on Github · Fly Docs

For a basic Ruby on Rails application running SQLite3 & Tigris and I cannot really get it to work?

Anyone else had success with this, and can share?
